Service Overview

This Informed Consent is for the undersigned’s (“you”, “I” or “your”) receipt of telehealth services (the “Services”) from Metabolic Health Medical Services, P.C., and Metabolic Health Medical Services, P.C., Inc. (“Metabolik Medical”) available through a technology platform made available by Metabolik Health, LLC (the “Platform”). The Services involve the use of electronic communications to enable healthcare providers at different locations to share individual patient medical information for the purpose of providing and improving patient care. The Services may include diagnoses relating to obesity, weight management assistance, remote prescribing, nutritional coaching and health information sharing.
Metabolik Medical’s providers (our "Providers") are an addition to, and not a replacement for, your primary care provider. Responsibility for your overall medical care should remain with your local primary care provider, if you have one, and we strongly encourage you to locate one if you do not. Before you give your consent to request Services, please be aware of how receiving care through our Platform differs from an in-person visit with your primary care provider or other specialist for the same care.
Provider(s) will be interacting with you solely via use of the Platform. As a result, all of the medical care and treatment you receive from such Providers will be provided via telehealth, and you will not be able to meet with your Provider in a physical location. You will be provided with the name, credentials, licensure/certification, and qualifications of the Provider who will be providing your care. You may contact your Provider(s) for follow-up questions by sending a message to your Provider(s) via our Platform. Your Provider will respond at his or her earliest convenience. However, a Provider should not be contacted in the case of a medical emergency, as noted below.

Summary Of Services
The following is a summary of the services we provide, which services, along with the terms and conditions on which they will be provided, are described in more detail in the following pages:
• You will have access to your doctor via our HIPAA compliant MetaboliK app.
• Remote visits will be conducted through secure telehealth platforms.
• Your membership fee covers all services we provide, which include remote physician visits and health coach appointments (each occurring as described in the "Frequency of Visits" section below) as well as remote patient monitoring.
• You will be responsible for the costs of medication, laboratory services, and the MetaboliK Kit (if not included in your plan), as well as any items purchased in our e-store.
• While our providers are able to help you navigate your insurance documentation to determine whether medications are covered, it is ultimately your responsibility to determine whether prescribed medications are covered by your insurance.
• We both agree to resolve any conflict through binding arbitration.
Medication Coverage
Prior to sign-up, members are strongly encouraged to schedule a complimentary call to assess medication coverage. By proceeding with membership purchase, members acknowledge that they have either confirmed their coverage or accept the risk of potential out-of-pocket expenses for medications. No refunds will be issued if a member signs up and subsequently discovers they have no coverage for prescribed medications.
Frequency Of Visits
• Monthly visits with the doctor (for all membership tiers).
• Biweekly visits with the health coach (for all membership tiers).
In the event of a no-show or a cancellation within 24 hours of a scheduled visit with either the doctor or the health coach, the visit will be considered used and will count towards your visit frequency.
Membership Tiers and Pricing
MetaboliK offers the following membership tiers:
· 1-month membership: $250/month*
· 3-month membership: $225/month*
· 6-month membership: $200/month*
· 1-year membership: $175/month*
*Cost of lab tests, medications, and items from the e-store are not included in the MetaboliK membership price.
All memberships include:
• Access to FDA approved GLP1 medications**
• Thorough initial evaluation, followed by monthly follow ups with Board Certified Obesity Medicine Specialist
• Biweekly visits with health coach
• Access to our MetaboliK app that automatically tracks your journey, making it easier to achieve your weight loss and health goals
• Personalized meal plan
• Access to our premium, healthy and high-quality products e-store
• Remote body composition monitoring for members with MetaboliK Kit/body composition scale. The MetaboliK Kit is included in the 1-year membership and available for purchase with other membership tiers
**Insurance coverage applies to FDA-approved medications. Coverage details may vary by plan; copays, coinsurance, and deductibles may apply. Medications are prescribed at the provider's discretion.
MetaboliK Kit
The MetaboliK Kit with scale to assess body composition (fat percentage, muscle mass, metabolic rate, and more) is included only in the 1-year membership plan. For other membership tiers:
• 1-month membership: $200 (no discount)
• 3-month membership: $150 (25% discount)
• 6-month membership: $100 (50% discount)
Discounts on the MetaboliK Kit are only available at the time of initial membership purchase. If the kit is purchased at a later date, the full price of $200 will apply.
Automatic Payments and Subscription Renewal
By selecting any of our plans, you agree to automatic monthly charges for the duration of your membership. Your payment method will be billed on the same date each month for the agreed-upon membership period.
- Billing Cycle: Your subscription will automatically renew each month on the anniversary of your initial sign-up date.
- Continuous Billing: After your initial membership period (1, 3, 6, or 12 months), your subscription will continue at the same rate and duration unless you change or cancel your plan.
- Payment Methods: We accept major credit cards, debit cards, and HSA/FSA cards (where applicable).
- Failed Payments: If a payment fails, we will attempt to process the charge again within 48 hours. If the second attempt fails, we will contact you for an alternative payment method.
- Price Changes: We reserve the right to adjust pricing. Any price changes will be communicated at least 30 days in advance.
- Cancellation: To avoid future charges, you must cancel your subscription at least 7 business days before your next billing cycle.
You authorize us to charge your payment method on file on a recurring basis for the membership plan you selected, including any applicable taxes and fees, until you cancel or change your subscription.
